Applying for Admission at Orangewood Academy

The packet below includes all the forms you will need to complete an application for admission to Orangewood Academy. Specific instructions for these forms and other aspects of the admission process are outlined below. All complete applications will be reviewed and responded to as soon as they are received.

Application Checklist

Please print legibly in ink to ensure the accuracy of our information.

Be sure to include all signatures (parent/guardian’s and student’s), on all forms.

  • Application for Admission form
  • Student Recommendation form (two required from any of the following: teacher, principal or pastor)
  • New Student Interview form
  • Student Questionnaire and Essays form (Grades 6-12)
  • Standardize testing results for the last two years
  • Grade Reports (Grades K-8) and/or Transcripts (Grades 9-12) for the last two years
  • Day Care Policy form. (Grades K-8)
  • Registration fee ($250)
  • Copy of your 1040 Income Tax return for previous year if applying for tuition aid
  • Acceptable Use Agreement form/Lifestyle Commitment and Student Citizenship Contract form
  • Birth certificate
  • Health examination
  • Immunization record
  • Oral health assessment (Grades K-1)
  • STAR Math & Reading Placement Test. Contact Mrs. Cuevas to set an appointment at
  • Foreign Student Application
  • Copy of Passport
  • Copy of TOEFL or iTEP test results
  • Interview with Principal. Contact Mrs. Munoz  to set an appointment at
  • Host Family Contract
  • AERC Translation of High School Transcripts
  • Guardian Authorization Form
  • Financial Policy for International Students
  • Copy of I-20 (transfer student only)
  • Copy of Student Visa (transfer student only)

Please note that I-20 visas will NOT be issued

for the following fall semester after the month of June.
